Why This Form Matters

This form is used to register patients with CKD stage 4–5 and those on dialysis in Hafr Al Batin. It helps HAB Health Cluster track, manage, and improve care for individuals with advanced kidney disease.

Who Should Be Registered

Patients with CKD stage 4 or 5 on OPD.
CKD 5 on replacement therapy , either Tx , HD or PD

INFORMATION SHOULD BE REGISTERED ON MONTHLY BASIS.

The CKD Form is used to record and update essential patient information in our Nephro OPD. The info will help us to have precise data about demographics, clinical status and forecast resources.
